The “twist and out” technique for removing an IOL can be tricky to master. Dr. Morgan Micheletti came up with a slight modification to Dr. Rahul Pandit’s original technique that he hopes will expand the adoption of this useful technique.
Instead of holding a cannula in his off hand, Dr. Micheletti bridges a 25-gauge cannula across the anterior chamber.
This modification frees up both hands to rotate the forceps, allowing for a smoother and more controlled rolling of the IOL against the cannula. As a result, the IOL can often be more tightly wrapped, meaning wound stretch is reduced and the IOL can be removed through a smaller hole.
