Intraocular lens (IOL) calcification in the presence of asteroid hyalosis is an uncommon late complication of cataract surgery. It is due to calcium-lipid deposits accumulating on the posterior surface of silicone IOLs. Here, Morgan Micheletti, MD, performs an IOL exchange with a pars plana approach to anterior vitrectomy in a patient who was experiencing decreased vision.
